Coordinated Care for Healthier Lives
Navigating healthcare and support systems can be difficult for individuals living with chronic health conditions. Project Hospitality’s Health Home program connects Staten Island residents with dedicated care managers who coordinate care and help individuals stay connected to the services they need.
Each member works closely with a care manager to develop a plan and access community resources from trusted providers that support long-term stability.

Our Health Home program provides comprehensive care coordination for individuals with complex medical and behavioral health needs.
Services We Provide:

Coordinate medical and behavioral health care
Schedule and attend medical appointments
Navigate hospitals, clinics, and healthcare systems
Access medications and treatment services
Connect with social supports such as food, benefits, and transportation
Identify community programs that strengthen long-term well-being
Wraparound Care
Project Hospitality’s care managers use the Wraparound Model, a collaborative approach that supports individuals through coordinated, individualized care.
Through this model:
Care is built around each individual’s unique needs and goals
Healthcare providers and community support work together as one coordinated network
Care managers help navigate services and connect individuals to trusted resources
Individuals are supported in staying engaged in care and managing their health over time
Tailored Support Transforms Lives
In 2019, Sue enrolled in our Health Home program because she was experiencing homelessness while managing multiple health conditions and struggling to find continuity in her care. With the support of a dedicated care manager, she coordinated medical appointments, secured transportation, and accessed food assistance services and housing resources.
By 2025, she had graduated from the program, was living in her own apartment on Staten Island, and was working full-time with benefits while remaining connected to her healthcare providers.
